Please have a digital copy of your child's birth certificate or passport ready to upload as proof of age.

Please have an official statement such as a utility bill or driver's license to upload as proof of address. Must show full address and name of one of the player’s parent or guardian.

The list of mandatory equipment for hockey includes:

• CSA approved helmet with face mask
• Hockey gloves
• Neck guard
• Elbow Pads
• Shin guards
• Shoulder pads
• Jock/Jill
• Hockey pants
• Hockey Skates
• Hockey stick. The end of the stick must be taped so it cannot fit through a face mask.

A thin layer beneath the equipment makes it more comfortable. Pajamas, yoga gear or thin long sleeve shirts and long pants prevent equipment from rubbing and straps from digging in to skin.

• Only one friend request for each player is permitted.
• Both players must be registered at the time of submission for their request to be considered.
• A friend request must be reciprocal – the parents of both players must submit a request by the deadline of Aug.15th, 2026 to hlcoordinator@leasidehockey.com
• While the LHA will make every possible effort to accommodate a friend request made before the deadline, a friend request can not be  guaranteed at any time. A friend request that can not be accommodated by the LHA is not eligible for a refund.

• Your player’s full name
• Their friend’s full name
• The age group of the players

We recognize that players enjoy having a familiar face on their team, however, in circumstances in which we are unable to accommodate a request, we are confident that all players will make new friends and enjoy the season regardless!
